The person in this position is the single point of accountability for ensuring overall recovery planning and service delivery for the residents on their caseload. Areas of responsibility include specific duties to be performed before, during and after recovery plan meetings, maintaining appropriate documentation and records to ensure compliance with applicable federal, state and facility rules, regulations, procedures, guidelines, and directives as well as maintaining an ongoing meaningful relationship with and always advocating for each resident. Facilitation, scheduling, and documentation of Recovery Plan Meetings. Advocating for the clients. Single point of accountability for ensuring overall recovery planning and service delivery for clients. Serve as liaison among all disciplines involved in providing services to clients to ensure that services are rendered. Review of client admission and clinical records for appropriate level of care and treatment and discharge planning.
Supervises employees by assigning work, reviewing progress, and assessing performance. Counsels’ family members to assist in understanding, dealing with, and supporting client or patient. Interviews clients, reviews records, and confers with other professionals to evaluate mental or physical condition of client or patient. Formulates or coordinates program plan for treatment, care, and rehabilitation of client or patient, based on social work experience and knowledge. Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan. Modifies treatment plan to comply with changes in client's status. Refers patient, client, or family to community resources to assist in recovery from mental or physical illness. Intervenes as advocate for client or patient to resolve emergency problems in crisis. Supervises and directs other workers providing services to client or patient. Counsel’s clients and patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.
This position involves professional investigative casework for the elderly or disabled adults alleged to be abused, neglected, or exploited. This requires the investigator to perform duties as assigned. Investigation of Reports of Abuse, Neglect, and Exploitation. Prepares for investigation. Commences investigation according to assigned response. Makes on-site and home visits to assess risk. Conducts interviews with subject of the report, i.e., vulnerable adult, guardian, or caregiver, possible responsible person. Collects evidentiary information through interviews, observations, and materials. Notifies State Attorney, Law Enforcement and other required individuals and appropriate agencies. Contacts collateral sources, i.e., State Attorney, Law Enforcement, medical personnel, neighbors, relatives, guardians, or other individuals having knowledge of the situation. Obtains available information from departmental background checks through HSN and other programs. Components to include Agency for Persons with Disability, Alcohol, Drug Abuse and Mental health, Community, Health Services, Economic Services, Licensure, and other agencies as appropriate. Provides or arranges for emergency placement, emergency medical treatment. Refers vulnerable adults to appropriate service providers as determined by the investigation which can be initiated at any point during the investigation. Engages service components through departmental staffing for the provision of services.
